2011 Outstanding Middle School Component Award Nomination Form

The purpose of the Ohio Middle Level Association Component Award Recognition Program is to identify and recognize schools that have implemented middle school concept components in an exemplary manner. It is the intention of OMLA that the recognized programs will serve as models for aspiring educators and provide model resources for schools striving to develop, refine or polish components of their middle schools. Recognized schools are open for visitations and encouraged to give presentations at the annual spring OMLA State Conference.

Outstanding Middle School Component Awards are given in the following areas:

Advisor/ Advisee                         Service Learning
Interdisciplinary Curriculum         Staff Development
Parent Involvement                     Student/Staff Recognition
Scheduling                                  Teaming

Nomination Criteria
*  Nominator must be a current member of the Ohio Middle Level Association.
*  All OMLA members are eligible to nominate a school for component awards.
*  Nominated school must be a current OMLA Institutional Member.
*  A school may be nominated for a component award in more than one area.
*  A school that has received a component award cannot be nominated for a component award in the same area for the following two years.
*  Teaming must be building-wide, not just one team in the building, for a Teaming Component Award nomination.

Application Criteria
*  Nomination must be received by the posted deadline to receive a Component Award Application.
*  Nominee will receive and must complete an OMLA Outstanding Component Award application.
*  A separate, specific application must be completed for each nominated area if school receives a nomination for a Component Award in more than one area.
*  Applicant provides documentation that demonstrates exemplary practices in the specific Component Award area.
*  Teaming must be building-wide, not just one team in the building, for a Teaming Component Award.
*  Applications must be received by the posted deadline.

Awarding the Honor
*  A committee of the Ohio Middle Level Association Executive Board members will review all applications.
*  Applicants which meet the criteria on paper are notified and a site-visitation team of OMLA members and/or consultants is scheduled to verify the middle level component practice.
*  Component Awards will be recognized at the OMLA annual state conference.

Nomination Form Deadline: November 19, 2010

A formal application will be mailed to qualified nominees.
